Program Manager - Home Care

Our client is a local, not-for-profit organisation and registered charity that provides community housing, support for at risk youth, home services for older Australians and disability support. They strive to alleviate everyday pressures and challenges for their clients and above all else, aim to give them independence.

You will look after a small, professional team of individuals who deliver a wide-reaching home services support program, to older Australians in our region. The role manages a Commonwealth funded budget that enables support services to promote independent living at home, as well as maintenance and improvements of client’s living situations and homes. The primary responsibilities of the role will include: 
  • Managing the growth and delivery of the Home Care Packages, including service delivery, information and reporting;
  • Growing the Home Services business by increasing the number of clients/recipients;
  • Manage and lead the Home Care team;
  • Intake of new clients and creating individual plans based off quality, evidence based and clinical governance advice;
  • Overseeing a caseload of approximately 85 to 90 clients (and growing);
  • Analysing the efficiency and effectiveness of the current service and implementing improvements;
  • Ensure the Home Care program complies with all legislative requirements including Aged Care Quality Standards, clinical governance and ISO9001;
  • Managing relationships with clients as well as third-party providers and subcontractors;
  • Participating in and developing the broader knowledge of home services programs and increasing clients; and
  • Contributing to the wider management team for the improvement and betterment of the organisation.
We are seeking an individual who has prior experience in a similar support and services role, for example; aged care, nursing and\or health environments. To be successful in the role, you will have a Diploma of Community Services or higher. We are looking for someone who will lead and manage the team and inspire them to even greater efforts, as we look to grow the service. Clinical qualifications are highly desirable, but not essential.
